Book Review: Work Permits in the Oil Industry

  The occurrence of work-related accidents imposes very large losses on every country each year. In developing countries, the costs of work-related accidents amount to about 5 to 10 percent of each factory's profit, and the cost that society pays directly and indirectly for accidents is about 2 to 3 percent of gross national product—roughly equivalent to one year's economic growth in developing countries. On July 6, 1988, 167 people lost their lives in the explosion and fire of the Piper Alpha platform in Britain. An inquiry and investigation into that accident was carried out under the leadership of Lord Cullen, in order to determine the root causes of this disaster and to offer recommendations for preventing the occurrence of similar events in the future.   [caption id="attachment_2578" align="aligncenter" width="334"] "Work Permits in the Oil Industry"[/caption] Lord Cullen published his report, which contained 106 recommendations, in December 1990. He concluded that one of the primary causes in the occurrence of this disaster was a failure in the functioning of one of the key safety-management systems—namely, the work-permit system. The International Association of Oil and Gas Producers (E & P Forum) assigned individuals to review Lord Cullen's report more closely. Their report, more than anything, emphasized one thing: the importance of safety-critical operating procedures, of which the work-permit procedure is one. This book is not only a guide for preparing and issuing work permits, but also provides information regarding the theoretical basis of various matters concerning the performance of repair work in the oil industry, as well as the requirements of credible international standards during the issuance of work permits and the performance of work, in order to prevent the occurrence of related accidents.
