An accident is defined as any unwanted, unplanned event that leads to harm to personnel or damage to equipment, installations, or the environment.
Accidents are usually complex; an accident may occur as a result of a sequence of more than ten events that can be regarded as its causes.
These events are classified into three categories: preliminary events, primary events, and intermediate events.
Preliminary events are anything that affects the primary events and sets the stage for the emergence of hazardous conditions. If the preliminary events of hazardous conditions can be eliminated, the accident cannot proceed to the next stage. In some sources, these events are referred to as the root and invisible causes of the accident. Long working hours and storing materials under unsuitable conditions are examples of preliminary events.
Primary events are the conditions or mechanisms that cause an accident to occur. The primary event can be likened to the spark that ignites a fire. These events are the main, visible causes of the accident.
Most control measures are taken, at the first step and in a basic way, at this stage.
Intermediate events are the triggers, some of which play a role in mitigating the accident and some in intensifying and shaping it.
For example, a safety valve in a pressurized system can, by reducing pressure, improve an overpressure condition in the system. Now, if these safety valves fail to operate when needed, they not only fail to prevent the accident but also intensify the hazardous conditions.
